Client Personal Information Collection Statement

It is often necessary for our current or prospective individual clients, or, where our clients are corporate or business entities, their individual representatives and employees (collectively referred to as "clients" and otherwise referred to as "client", "you" or "your") to provide to UNISON Insurance Broking Services Private Limited* (hereinafter referred to as " UNISON", "we", "our" or "us",) personally identifiable data about yourselves ("Personal Information")in connection with our business process execution, including delivery of services and/or products, preparation of proposals, provision of quotations, arranging insurance cover, managing claims, client relationship management and conducting internal conflicts checks. Such Personal Information may include information or data provided by you or other parties or from other source, and may include, but is not limited to, historical or existing data and/or data to be collected in the future. Such Personal Information may be subject to applicable data protection, privacy and other similar laws and may include copies and other details of identity documents, proof of address and other contact details, religious, philosophical or political affiliations, information concerning age, marital status, racial or ethnic origin, education, genetic or sexual life, physical or mental health or medical condition/diagnosis, dietary preference, commission or alleged commission of any offence or proceedings for any offence committed or alleged to have been committed, the disposal of such proceedings or the sentence of any court in such proceedings.

When you provide UNISON with Personal Information relating to your employees, dependents and/or other individuals that you represent, you warrant that you have obtained the consent of such parties for the collection, use and processing of their Personal Information by UNISON.

Personal Information you provide will be collected, used and otherwise processed by UNISON for the following purposes:

• Client relationship management procedures, including any potential conflict checks as may be required;
• The delivery of services or products to the client;
• Those purposes specifically provided for in any particular service or product offered by UNISON;
• Conducting marketing and client profiling activities in connection with insurance and related services and products (including those provided by UNISON and selected third parties for the purpose of improving our services to the client or that we think may interest the client);
• Credit assessments and other background checks of the client as UNISON may determine to be necessary or appropriate;
• UNISON’s internal record-keeping;
• Collection of outstanding payments from clients;
• Prevention of crime (including but not limited to fraud, money-laundering, bribery);
• Meeting any legal or regulatory requirements relating to UNISON's provision of services and products and to make disclosure under the requirements of any applicable law, regulation, direction, court order, by-law, guideline, circular, code applicable to UNISON; and
• Purposes relating to any of the above (including but not limited to research, benchmarking and statistical analysis).

Collection and Disclosure:

Personal Information provided to UNISON will generally be kept confidential but you hereby consent and authorize UNISON to collect, provide or disclose your Personal Information for the purposes stated in paragraph 2 above from or to

• Any person to whom UNISON is compelled or required to do so under law or in response to a competent or government agency;
• UNISON;
• Government agencies and industry regulators;
• UNISON’s auditors, accountants, lawyers or other financial or professional advisers; and
• Such sub-contractors or third party service or product providers as UNISON may determine to be necessary or appropriate, in accordance with paragraph 2 and paragraph 7.
• Such person(s) as you may instruct or require.
• Unison Edge Plus App's use and transfer of information received from Google APIs to any other app will adhere to Google API Services User Data Policy, including the Limited Use requirements

You further consent to provide, and for your employer, insurer(s), health maintenance organizations, agents and/or third party service or product provider(s) to provide to UNISON your Personal Information for the purposes set out in paragraph 2 above.

Failure to provide such Personal information may result in UNISON being unable to provide clients and you with the services and/or products requested.

Safeguards:

UNISON confirms that UNISON has implemented the appropriate administrative and security safeguards and procedures in accordance with the applicable laws and regulations to prevent the unauthorized or unlawful processing of your Personal Information and the accidental loss or destruction of, or damage to, your Personal Information.

Data Transfer:

Where UNISON considers it necessary or appropriate for the purposes of data storage or processing or providing any service or product on our behalf to you, we may transfer your Personal Information to An Affiliate or third party service or product providers within or outside the country in which UNISON is established, under conditions of confidentiality and similar levels of security safeguards.

Your Rights of Access and Correction:

You have the right to request access to and correction of information about you held by UNISON and you may:

• Check whether UNISON holds or uses your Personal Information and request access to such data;
• Request that UNISON correct any of your Personal Information that is inaccurate, incomplete or out-of-date;
• Request that UNISON specify or explain its policies and procedures in relation to data and types of Personal Information handled by UNISON;
• Communicate to UNISON your objection to the use of your Personal Information for marketing purposes whereupon UNISON will not use your Personal Information for these purposes; and
• Withdraw, in full or in part, your consent given previously, in each case subject to any applicable legal restrictions, contractual conditions, reasonable internal policies/procedures, a reasonable time period (in accordance with applicable laws) as well as, in the case of an access request, a reasonable fee (where permitted under applicable laws and as UNISON may notify you in writing upon receipt of your request).

Written requests for access to Personal Information or correction and/or deletion of Personal Information or for information regarding policies and procedures and types of Personal Information handled by UNISON may be sent to UNISON.

Privacy Policy for Google Fit/Apple Health Kit Integration

If you do not wish to receive promotional information from us, you can, at any time, choose to opt out of receiving our promotional newsletters. To unsubscribe from our newsletters, you can follow the directions included at the newsletter.
• If, at any time, you want to access, update, modify or eliminate any of your account information in ‘UNISON EDGE PLUS App’, you may do so by clicking “Your Account” and then review / change your sensitive personal information as you want it to be.
• ‘UNISON EDGE PLUS App’ may, in future, request other optional information from you to customise ‘UNISON EDGE PLUS App’ to deliver personalised service to you.

What Information we collect about you:

We do not automatically collect your personally identifiable information. However, you might provide us with your personal and demographic information – including your Name, Gender, Age, Address, Phone number, Email address and your medical prescription(s), in the following ways –

• As a New Customer, when you sign in to account at ‘UNISON EDGE PLUS App’;
• As an Existing Customer, when you update your existing profile on ‘UNISON EDGE PLUS App’;
• During your visit to ‘UNISON EDGE PLUS App’, we may automatically receive technical information about your computer / device, including your IP address, your computer operating system, time-zone, browser type and browser plug-in details, due to the communication protocol settings on the Internet.
• When you browse ‘UNISON EDGE PLUS App’, we may also collect information regarding the pages you viewed, the web addresses from which you arrive or click through to, time spent on certain pages, download errors and page response times. All these information help us analyse Customers’ trends and preferences, and thus assist us in improving our service.
• For giving an enhancement to your health Information, ‘UNISON EDGE PLUS App’ also collects your Google User Data in the Method and Scope described below –
• Google Fit Authentication – UNISON EDGE PLUS Google oAuth Consent Flow
• Google User Data – ‘UNISON EDGE PLUS App’ collects your following data from your Google Account –
• Steps Data as available in your Google Account
• Sleep Data as available in your Google Account

How we Use the Information collected from you:

10.1 We will collect and retain your personally identifiable information only to the extent that it is necessary to fulfil our services to you. Any Personal information submitted by you to ‘UNISON EDGE PLUS App’ or collected by us will be used ONLY for the purposes mentioned below –

• To carry out our obligations arising from your requests for our products and services;
• To improve ‘UNISON EDGE PLUS App’ functionality, and to customize your future experience with us;
• To make sure that ‘UNISON EDGE PLUS App’ content is presented in the manner that is most effective for you;
• To communicate with you about any changes to our products and services.
• To verify your identity and perform checks to prevent fraud.

The Information which is Collected and Generated is stored in encrypted format on our Secured Backup Systems because of its size, complexity, and subsequent usage of it by you and the Authorized UNISON Personnel. Note that, we do not store your Credit Card details, Debit Card details, Google Account User details.

With whom we share your Information:

• All your Information in ‘UNISON EDGE PLUS App’ is accessible only to YOU and the Authorized UNISON Personnel, no other entity has access to this Information.
• We do not sell, rent, or exchange your any personally identifiable information that you provide to us or is collected through ‘UNISON EDGE PLUS App’, with any third party for commercial reasons or otherwise.

11.3 As with any business, it is possible that as our business develops, we might sell or buy other businesses or assets. In such transactions, User / Customer information is typically one of the transferred business assets. Accordingly, in the event that ‘UNISON EDGE PLUS App’, or substantially all of our assets, is acquired by a third party, such information may be one of the assets that is transferred or acquired by a third party. Under such circumstances, ‘UNISON EDGE PLUS App’ would, to the extent possible, require the acquiring party to follow the practices described in this Privacy Policy. You acknowledge that such transfers may occur, and that any acquirer of ‘UNISON EDGE PLUS App’ may continue to process your personal information as set forth in this Privacy Policy.

Who has access to your Information:
